Care Management

Our Care Management department facilitates and promotes an integrative, high quality, efficient, and educational approach to service delivery. As an engaged team, integral to the daily operations of the medical center, we balance an empathic approach to patient / family interaction with the sound knowledge of the business of healthcare and serve as champions of change to continually improve the process of care management for the benefit of our patients. We promote staff involvement in improving and developing standards of care and in influencing change within the profession.

Our Facilities

In 2009, the Care Management Department was restructured in an effort towards the development of a new model that ensures greater responsiveness to the throughput needs of the medical center and the clinical utilization / discharge needs of our patients. Changes occurred at all levels within the department including leadership, staffing, process management, and integration between care management and social work. The department focus includes length of stay, with emphasis on the extended stay cases, emergency department care management / throughput, clinical advancement / education for professional staff, enhancing community service / agency collaboration, and reduction of inpatient medical necessity denials.

Staff Quality

Key to the success of the Care Management department was the development of a high performing, integrated team of nurse care managers and social workers, supported by a core of administrative support staff. Integration efforts included a redesign of the department model, team building, intensive staff recruitment efforts, leadership restructure, development of a shared vision statement, and redesign of the discharge planning unit.
